Au cœur de la psychothérapie se déploie un processus dans lequel psychologue et client.e tentent ensemble d’apaiser des expériences porteuses de souffrance. Pour ce faire, les psychologues articulent leurs interventions autour des expériences émotionnelles à l’origine de cette souffrance. Or, ceci peut s’avérer particulièrement complexe auprès de client.es qui éprouvent une difficulté à reconnaître et décrire leurs émotions, c’est-à-dire auprès de client.es hautement alexithymiques. This article presents an identification, a classification and the development of a questionnaire to measure feedback interventions used after an individual psychological assessment. Given the lack of research on feedback interventions in this context, this researsh is based on feedback litterature in related fields and on the expertise of potential assessment practitioners. The use of the questionnaire after 254 feedback sessions allowed the verification of a model derived from the proposed classification. Results identify six feedback interventions and a partial confirmation of the anticipated theoritical model.

AbstractThis article deals with physical and sexual violence, isolation, high suicide rate and drug and alcohol abuse that often tear the social and family fabric of many Native communities of Canada.Substance abuse has long been identified as one of the root causes of various disorders found among Natives. We observed that this abuse is really more a symptom than a cause after looking at it from an historical and social perspective.With this knowledge and awareness of past heritage, intergenerational transmission of violence and substance abuse as well as the potential for healing, the local representatives of the Nutashkuan community have, with the help of psychologists, set up a healing project based on therapeutic expeditions on the Nitassinan river territory.At the start, this project was concerned with the problematics of substance abuse, but it has gradually been transformed into a co-creation environment based on the search for solutions.Today, the focus is no longer aimed at the destructive effects of substance abuse, but rather on developing well-being and protective elements which find their expression in family harmony and social peace.

As is all medicine, psychiatry is an art. Therefore its hypotheses and knowledge are tied to its place in a social context of professional power plays and competition. In the last fifteen years, while public psychiatry has seen its responsibilities grow to cover new populations and issues, the profession has not moved towards identifying social causes for mental health problems. Instead there has been clinical uncertainty, due to the breakdown of the border between health and social work. Nevertheless, the profession's current representations of the social are undeniably more complex, all the while testifying to the persistence of a professional culture seeking to maintain, via demands for legal and political reform, the distinction between the social and its symbolic manifestations.

When studying medicine in relation to social control, the sociological analysis often forgets to mention the values underlying medicalization. The development of American psychiatry, between 1950 and 1980, enlightens the driving force exerted by the individual's defense in promoting mental health. The principle of individuality has offered a new perspective on patients' rights and on the legitimation of psychic disorders. However, it also initiated a well-being requirement, acquired through the treatment of pain, and surpassing individual accomplishment. This moral pattern feeds the medicalization and also subjugates it to the contradictory imperative of the subject's compensation and protection. As this article attempts to demonstrate, psychiatry, as a psychological medicine, explains this tension in greater detail. The ambivalence of health requirements affects the organisation of professional morphology, influences the evaluation of psychiatric categorization, and modifies its appraising role.

Objectives This article advocates in favor of increasing the accessibility of psychological services in primary health care by focusing more specifically on the relevance of including psychologists in family health teams in primary care in Ontario.Methods The authors present their advocacy from two levels of information: 1) the main results of a demonstration project funded by the Primary Health Care Transition Fund (PHCTF) in which psychologists were integrated into family practices; and 2) experiences and general observations drawn from the combined experiences of the authors from the last decade regarding the inclusion of psychologists in primary care.Results Main results from the demonstration project: 1) highly prevalent mental illnesses (anxiety & mood disorders) are amenable to psychological interventions; 2) psychologists and family physicians are natural and complementary allies in primary care; 3) the cost of integrating psychologists to provide psychological interventions can be off-set by a reduction in physicians' mental health billing. Main observations drawn from authors' combined experiences in primary care: 1) relatively few psychologists work in family health teams in Ontario; 2) most non-pharmacological mental health interventions in primary care involve generic counselling, problem solving, educational groups, and linking to community resources; 3) lack of understanding of the difference between evidence-based psychological treatments and generic counselling; 4) many multidisciplinary clinics unfortunately benefit from only one type of non-medical mental health professional as part of their team to see all cases, independent of the level of complexity on the patient's side, and independent of the level of expertise or supervised training on the provider side; 5) multidisciplinary teams in primary care need various mental health professionals to cover for a wide range of presenting problems and levels of complexity/co-morbidities.Conclusion Our demonstration project combined with our continued clinical experience in primary care is consistent with the scientific literature on the topic of psychological services in primary care. Common mental health problems are highly prevalent. Societal costs are high. Accurate diagnosis is crucial. There are psychological treatments proven to work, and that are cost-effective. We can build on existing multidisciplinary teams and structures in place and also innovate by finding creative mechanisms linking the public and private sectors such as in the Australian primary care system. Interdisciplinary teams should integrate a judicious mix of pertinent skills with the right balance of varying levels of competencies to efficiently address varying levels of problem complexities and co-morbidities. At the moment, there is a gap to be filled in multidisciplinary primary care teams and psychologists are already extensively trained and available to fill this gap.
